Generally you encounter this sort of error when your browser.double reads the save button and attempts to send the form twice in a very short span of time (like milliseconds).

TNG locks the record during edit or add operations, so the second submission is rejected with the SQL error.

Don't panic. Just check to see if the edit or insertion was completed; if so, everything is right with the world. I had an editor panic when she encountered a similar event because she thought she had broken the program.
